SHETE ENT CLINIC IS CLOSING

It is with a heavy heart that I announce the closure of my medical practice, effective May 31, 2023. I am truly grateful for the trust and support you have shown me over the years, and I am privileged to have served as your healthcare provider.

As I close my practice, I want to ensure that your transition to new healthcare providers is as seamless as possible.  You or your new healthcare provider has full access to your records.

• Request records online: www.morganrm.com > Patient Records Requests > Request My Medical Records
• There is a nominal processing fee for all medical records requests regardless of who is requesting the records.
• Email us at medical@morganrm.com or call 833-888-0061 if you have questions or need a form mailed to release records for those that don’t have access to a computer.

Finally, I want to express my gratitude for the trust you have placed in me as your doctor. It has been my honor to care for you and your families, and I wish you all the best in your future health endeavors.
